Community Health Worker Intern

About the Role

• This internship is based in our Belleville, IL primary care setting, offering exposure to whole-person care.
• Gain valuable experience and insights into behavioral health as you work toward your degree.

Key Responsibilities

• Help facilitate integrated primary and behavioral health care.
• Respond to patient inquiries.
• Help patients understand care processes and the financial aspects of behavioral health care, linking them to resources as needed.
• Screen clients for social determinants of health and make service referrals.
• Facilitate communication among patients, their families, and providers.
• Manage follow-up visits and ensure continuation of care.
• Arrange for language interpretation as needed.

Requirements & Tech Stack

• Basic computer skills including MS Office, Adobe Acrobat, and an electronic medical record.

Qualifications

• Current enrollment in a Community Health Program.
