Caregiver (Flexible/Float)

About the Role

Sevita, through its Mentor Community Services program, offers community-based services for individuals with intellectual and developmental disabilities. This role is a Full Time Float position for East Valley Behavioral Health Group Homes with a pay rate of $17.50. The position involves supporting therapeutic and behavioral plans in community, vocational, or in-home settings, providing assistance and strategies for individuals to live as independently as possible.

Key Responsibilities

• Provide assistance with daily activities including socialization, behavioral development, personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately document progress and activity; review records and logs to stay up-to-date with changes in service plans.
• Provide clinical expertise, potentially including knowledge for specialized populations such as intellectual and developmental disabilities, brain injury, emotional or behavioral disability, or expertise in special disciplines like behavioral support, early intervention, crisis intervention, or others.
• May accompany individuals to medical appointments and relay orders and information to and from medical providers.
• Travel between sites, to individual’s homes, and may accompany them into the community.

Requirements & Tech Stack

• Must be 21 years of age.
• Must have a valid driver’s license in good standing.
• Must be able to obtain a level one fingerprint card.
• States may have additional requirements.
• Strong attention to detail, organizational skills, and the ability to multi-task to meet deadlines.
• Excellent communication skills.
• A reliable, responsible attitude and a compassionate approach.

Qualifications

• High school diploma or equivalent required.
• Six months of experience in human services preferred.
