About the Role
Clarvida is seeking a Behavioral Health Care Coordinator to support children, adolescents, and families through community-based behavioral health services. This role involves providing case management support, coordinating care, and advocating for clients by connecting them to essential resources and services for stability, growth, and improved outcomes. This is a hands-on, client-facing position requiring strong communication skills, flexibility, and the ability to work collaboratively with treatment teams in a community-based environment.
Key Responsibilities
- Assist in the case management of children and adolescents
- Actively support clients and families in achieving treatment and service goals
- Collaborate with multidisciplinary treatment teams to develop and evaluate care plans
- Serve as an advocate and resource connector for individuals and families
- Coordinate services and referrals to community organizations
- Maintain accurate, timely, and compliant documentation
- Work flexible hours and across multiple locations as required by program needs
Requirements & Tech Stack
- Bachelor’s degree in a Human Services discipline from an accredited college or university
- Experience working with children and/or adolescents in a therapeutic, community-based setting
- CPR and First Aid certification (or willingness to obtain prior to working with clients)
- Strong organizational, communication, and teamwork skills
- Ability to work flexible schedules and travel between service locations
