Key Responsibilities
- Provide on-site medical care to skilled nursing and long-term care patients.
- Manage acute and chronic conditions.
- Provide post-hospital care.
- Coordinate with nursing, therapy, specialists, and facility leadership.
Requirements & Tech Stack
- Oregon NP license, or eligibility to obtain one.
- National certification and DEA eligibility.
- Strong clinical judgment and communication skills.
- Efficient, independent, and highly motivated.
- SNF experience is helpful but not required.
