Facility Medical Coder

About the Role

The Facility Medical Coder is a full-time remote role responsible for accurately coding inpatient and outpatient facility encounters. This involves reviewing medical records and documentation, assigning appropriate codes for diagnoses, procedures, and services, and ensuring codes support correct reimbursement and compliance. The role contributes to reducing denials, improving claim accuracy, and supporting efficient revenue cycle operations.

Key Responsibilities

• Accurately code inpatient and outpatient facility encounters using standard coding guidelines, payer policies, and regulatory requirements.
• Review medical records and documentation.
• Assign appropriate codes for diagnoses, procedures, and services.
• Validate medical necessity.
• Identify documentation gaps.
• Collaborate with clinical and revenue cycle teams to resolve coding issues.
• Work with AI-driven coding tools.
• Participate in quality audits.
• Stay current with coding updates, payer rules, and regulatory changes.

Requirements & Tech Stack

• Proficiency in Medical Terminology and Medical Coding systems (e.g., ICD-10-CM/PCS, CPT, HCPCS) and related guidelines.
• Familiarity with U.S. healthcare payer rules, compliance requirements, and denial management practices.
• Comfort using coding software, EHR systems, and AI-enabled tools to support coding and quality review.
• Strong attention to detail, analytical skills, and ability to meet productivity and accuracy benchmarks in a remote environment.
• Effective written and verbal communication skills and ability to collaborate across global, cross-functional teams.

Qualifications

• Demonstrated medical coding experience with facility-based records, including inpatient and outpatient encounters.
• Strong foundation in Health Information Management, with knowledge of documentation standards and revenue cycle processes.
• Relevant certification such as RHIT or equivalent credential in health information or coding is preferred.
