Field Nurse

About the Role

FreedomCare is seeking a Case Manager, RN for its Oregon team. This is a field-based, RN position requiring regular travel throughout an assigned Portland, Oregon territory to conduct in-home visits and support member care.

Key Responsibilities

• Conducts initial in-person clinical assessments and coordinates service activation, ensuring all required forms and standard procedures are followed.
• Develops, implements, and manages patient's activity plan(s), making revisions and documenting changes as needed.
• Completes all client documentation accurately, timely, completely, concisely, and legibly in accordance with agency policy and procedure.
• Actively coordinates client care through communication and written documentation to physicians, other disciplines, and community resources.
• Provides ongoing coaching to drive effective utilization of established best practices, encourages PCA education, and connects families to appropriate resources.
• Reviews caregiver notes daily and appropriately addresses changes in the consumer’s condition or environment.
• Ensures continuous compliance with all regulatory standards and company policies and procedures.
• Performs other job-related duties as may be required or directed by the supervisor.

Requirements & Tech Stack

• Valid and Active Registered Nursing License issued by the Oregon Board of Nursing.
• Ability to articulate cases through effective summaries.
• Ability and flexibility to travel (usually by car) within an assigned geographic area.
• Valid driver’s license and car insurance required.
• Proficient in common software and web-based applications (e.g., Case Management Systems, MS Office) with the ability to teach others.
• A passion for providing a high level of customer service, including the ability to show empathy, active listening, and patience.

Qualifications

• Diploma from an accredited nursing program. New graduates welcome!
• Home care case management (is a plus)
• Care coordination experience (is a plus)
• Bilingual (is a plus)
