About the Role

Safe Life Home Health Care, LLC is seeking a Home Health RN Case Manager to join our award-winning 4-star clinical team. This full-time or part-time position is located in Chicago and its Suburbs, IL. We offer a competitive salary range of $70,000 - $80,000 annually plus comprehensive benefits. We provide comprehensive training and welcome both new graduates and experienced nurses. International healthcare professionals are also welcome and will receive immigration assistance, including H1B visa sponsorship and employment-based green card applications for qualified candidates.

Key Responsibilities

• Clinical Care Management: Conduct comprehensive patient assessments, develop individualized care plans, and coordinate multidisciplinary healthcare services.
• Patient Education: Provide expert guidance on disease management, medication compliance, and health maintenance strategies.
• Documentation Excellence: Utilize WellSky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments.
• Care Coordination: Collaborate effectively with physicians, specialists, and healthcare team members to ensure seamless patient care.
• Quality Assurance: Monitor patient progress, adjust care plans as needed, and ensure adherence to clinical standards and regulatory requirements.
• Family Support: Educate and support family members and caregivers in patient care techniques and safety protocols.

Requirements & Tech Stack

• Licensure & Certifications:
• Valid Illinois Registered Nurse license
• Current CPR certification
• Valid driver's license and auto insurance
• Experience & Skills:
• Open to new graduates and experienced nurses - comprehensive training provided.
• Willingness to learn WellSky Kinnser EMR system (training included).
• Strong clinical assessment and critical thinking abilities.
• Outstanding written and verbal communication skills.
