About the Role
This internship offers valuable experience and insights into behavioral health within a primary care setting. It provides exposure to whole-person care and is based in Belleville, IL.
Key Responsibilities
- Facilitate integrated primary and behavioral health care.
- Respond to patient inquiries.
- Help patients understand care processes and financial aspects of behavioral health care, linking them to resources as needed.
- Screen clients for social determinants of health and make service referrals.
- Facilitate communication among patients, their families, and providers.
- Manage follow-up visits and ensure continuation of care.
- Arrange for language interpretation as needed.
Requirements & Tech Stack
- Basic computer skills including MS Office, Adobe Acrobat, and an electronic medical record.
Qualifications
- Current enrollment in a Community Health Program.
